- A letter of request on company letterhead.
- Company information (full address, phone number, email, and website).
- Notarized Deed of Incorporation and all amendments (if any).
- Information on company executives as stated in the notarial deed (including personal information, photos, and national ID cards).
- Designation of one permanent representative, along with a photo and ID card.
- Decision Letter (SK) Granting PPIU & PIHK Licenses (for companies that already hold a PIHK license).
- Tourism Business Registration Certificate (TDUP).
- Written recommendations from two PPIU and/or PIHK organizations that are members of GAPHURA.
- Company logo file (digital format).
- The Integrity Pact signed by the company’s leadership.
